DWC-041: Employee's claim form for a work-related injury or illness; must be filed within one year of the date of injury. 

DWC-001: Employer's report of injury or illness; must be submitted to the insurance carrier within 8 days of the absence or notice of injury.

DWC-053: Employee request to change a treating doctor.

DWC-048: Request to get reimbursed for travel costs related to medical treatment.
